Ware Medical Associates PC (“we”) is a covered entity under HIPAA. This page summarizes how we handle protected health information (PHI) on our website and Language Help tools.
Website
Public pages collect only what you voluntarily send (for example, a new-patient interest form). Do not put clinical details in public forms — use the Patient Portal or call the office.
Language Help
Voice interpretation may process spoken content that includes PHI. Audio and transcripts are processed to deliver the translation and are not stored long-term by default (session memory only). Vendors operating under Business Associate Agreements process data on our behalf.
Your rights
You may request access, amendments, restrictions, and an accounting of certain disclosures as allowed by law. Contact the office at 610-485-6700 or ask for our Privacy Officer.
